Umbria & Lazio hill towns

Slow towns, real budgets.

Civita di Bagnoregio, a hill town in Lazio

Orvieto, Todi, Spoleto, Civita di Bagnoregio: historic towns on tufa and hilltops, an hour or two from Rome but a world away in pace. This is the Italy of morning markets and passeggiata — at prices that have not been discovered by the international market the way Tuscany has.

Towns retirees shortlist

Retirees we speak to typically shortlist Orvieto and Todi (good services, rail access), Spoleto (culture, a real year-round town), and smaller villages around them. The trade-off is simple: the smaller and cheaper the town, the fewer services it has.

What it costs, honestly

Healthcare access

Orvieto, Todi, Spoleto and Perugia have hospitals; for specialist care you will likely travel to Perugia, Terni, or Rome. English-speaking doctors exist but are scarcer than in expat hubs — this is a real consideration if you have ongoing conditions.

Day to day

Smaller comuni mean shorter lines and more personal treatment at the town hall — but also less English, fewer appointment slots, and processes that rely on showing up in person. A car is close to essential in most of these towns.

Who this fits

Retirees who want the 'real Italy' at a manageable cost, are willing to learn Italian, and do not need big-city healthcare on their doorstep.

Worth knowing: The 7% foreign-pensioner flat-tax regime mostly covers smaller municipalities in the south — but a designated list of earthquake-zone comuni in Umbria and Lazio also qualifies. Do not assume either way: check your exact town before the regime shapes your plan.
